Retinal Toxicity Due to Antimalarials: Frequency and Risk Factors
نویسندگان
چکیده
Objective: To study the frequency of retinal toxicity and associated risk factors in a cohort of patients treated with antimalarials and seen at a tertiary level hospital. Material and methods: Retrospective study of 40 patients treated with antimalarials, who were referred to ophthalmology for the study of retinal toxicity during 2011. Data collection included type of antimalarial prescribed, daily and cumulative doses, presence of rheumatic disease, corticosteroid use, associated diseases and ophthalmologic examination. Retinal toxicity was confirmed if two of the following tests were altered: fundus examination, visual field with a macular pattern, changes in spectral domain optical coherence tomography (SD-OCT) and full-field electroretinography (ERG). Results: Toxic retinopathy was detected in 13.1% of patients (95% confidence interval 5%–21%) and a trend for a higher risk was observed in case of chloroquine (CQ) treatment. Among the patients with retinopathy, the mean cumulative dose was 229 g for CQ and 111 g for Hydroxychloroquine (HCQ), and the mean daily dose of CQ was 250 mg and 333 mg for HCQ. Arterial hypertension had a statistically significant effect on retinopathy development. Conclusions: Toxic retinopathy defined by ophthalmological evaluation was detected in 13.1% of patients. A trend for a higher risk was observed in case of chloroquine treatment. © 2012 Elsevier España, S.L. All rights reserved. Toxicidad retiniana por antipalúdicos: frecuencia y factores de riesgo

متن کامل(Hydroxy)-chloroquine retinal toxicity: two case reports and safety guidelines.
Retinal toxicity as a result of antimalarial therapy has been reported for many years. Retinopathy may be divided into reversible premaculopathy and irreversible true retinopathy. Risk factors for the development of toxicity are daily dosage related to body weight, total drug dosage and the specific drug used. The daily dosage of chloroquine should not exceed 4 mg/kg lean body weight a day. Exc...
متن کاملRetinal Toxicity in Patients Treated With Hydroxychloroquine: A Cross-Sectional Study
Hydroxychloroquine (HCQ) is an antimalarial medication that can also be used to treat autoimmune diseases. However, it can produce irreversible changes to the retina that lead to visual impairment. The aim of this study was to determine the proportion of patients treated with HCQ who develop retinal toxicity and the risk factors for the development of HCQ-induced retinal toxicity among Iranian ...
متن کاملRetinal toxicity associated with chronic exposure to hydroxychloroquine and its ocular screening. Review
Hydroxychloroquine sulfate (HCQ, Plaquenil) is an analogue of chloroquine (CQ), an antimalarial agent, used for the treatment of systemic lupus erythematosus, rheumatoid arthritis and other autoimmune disorders. Its use has been associated with severe retinal toxicity, requiring a discontinuation of therapy. Because it presents potential secondary effects including irreversible maculopathy, kno...
متن کامل[Quinacrine in the treatment of cutaneous lupus erythematosus: practical aspects and a case series].
Hydroxychloroquine and chloroquine are antimalarials used as first-line treatment of cutaneous lupus. Quinacrine is not often employed by Spanish physicians due to a lack of information about its use and the fact that it is not marketed in Spain. It is effective in monotherapy or in combination therapy with other antimalarials. One of the advantages of quinacrine over chloroquine and hydroxychl...
